As Trooper Brady was responding to a call for assistance to a domestic violence dispute, he was involved in a motor vehicle accident when another car pulled into his path. Trooper Brady swerved off the roadway striking a bridge abutment. The car burst into flames trapping him inside. Trooper Brady was assigned to Post 16 in Henderson.
He was survived by his wife, daughter, and son, who later became a Kentucky State Police Trooper, retiring in 1991.
The Trooper Mack E. Memorial Highway is named in his honor in Henderson County.
