Firefighter Hoffman was riding tailboard on a pumper that was responding to a fire in a television cabinet factory. During the response, the pumper collided with a flatbed truck. Firefighter Hoffman was thrown from the truck. He was transported to Deaconess Hospital where he died seven hours later. The accident also claimed the life of Firefighter Fred Barnard and injured Firefighter Francis Titzer. Firefighter Hoffman's son retired from the Evansville Fire Department.
