While working at a fire on Olive Street, the company that Firefighter Green was assigned to was working on the roof. The fire blocked their escape and they were trapped. They dragged their hoseline to the edge of the roof and prepared to scale down the hoseline to a ladder. Firefighter Green slid down the hoseline to the nozzle. However the ladder was not in position and was still several feet away. He was overcome by smoke and fell approximately sixty feet and died at the scene. Once the ladder was in position, the four remaining firefighters on the roof utilized the same method and made it to safety.
