Incident

Firefighter Smith was standing on the rear step of Hose Reel #7 while on a response. As they passed over the car tracks at Eleventh and Clark Avenues, he lost his grip and fell. The engine following the hose reel ran over his right leg. He was taken to City Hospital and then to Saint Luke's where his leg was amputated. He never recovered and died three days later.
