While preparing to extinguish a small fire in a basement at 714 Locust Street, the chemical extinguisher that Captain O'Toole was about to use exploded. He received severe trauma to his chest and died a short time later. Captain O'Toole was credited with saving numerous lives over his career.
In 1994, the St. Louis Fire Department christened a thirty-two foot, twin-engine fireboat the "Philem O'Toole" in his honor.
